Sports Medicine & Lameness Evaluations
Sport medicine is the heart of B.W. Furlong & Associates, with Dr. Furlong having served as the US 3-Day Eventing team veterinarian for the past five Olympic Games. In-depth evaluations are performed to keep horses at peak performance. Lameness exams performed at the clinic are improved by the use of a controlled surface jog strip and lunging/riding ring. Diagnosis is also facilitated by the use of digital radiography, digital ultrasound, and the lameness locator. Complex lameness cases may be further evaluated by the use of standing MRI and nuclear scintigraphy. Other therapies include shockwave therapy, IRAP, stem cell therapy, Tildren and mesotherapy.
Prior to the investment of purchasing a new horse, a complete pre-purchase examination is highly recommended. The format can be tailored to the individual needs of the client, but usually involves a thorough physical examination from head to tail. The horse is observed for soundness, and digital radiographs are encouraged to establish a baseline of skeletal changes. Reproductive exams, ultrasound exams, and upper airway endoscopies are among the ancillary procedures also available.
Internal medicine services are provided as both an in-patient and out-patient service by Dr. Rachel Gardner, a board certified internal medicine specialist. These services include evaluation of horses with complex respiratory, gastrointestinal, liver, kidney, neurologic and ophthalmic disorders. Evaluations are provided for subtle abnormalities, such as changes in performance, or weight loss. Intensive care with 24 hour monitoring is available for adults and neonates, and an isolation stall is available for treatment of horses with potentially contagious diseases. Cases are accepted on an appointment and emergency basis.
B.W. Furlong & Associates provides first rate veterinary care for routine and emergency needs. Preventive medicine and routine patient care, including vaccinations, deworming, Coggin’s tests, and health certificates, are performed regularly by a number of our veterinarians.
Radiography and ultrasonography are two of the staples of veterinary medicine, and B.W. Furlong & Associates has been using these practices for years. The clinic was one of the first in the northeast to ever begin using digital radiography. The practice has state of the art equipment, and imaging can be immediately archived and digitally shared to any other practice around the world.
Since its inception in 2004, Equine MRI of NJ has become the busiest magnet in the United States, serving over 200 patients a year. Each study is performed using sedation, which eliminates the need for general anesthesia and the risks associated with it. MRI is the best modality to evaluate the foot (the source of 80% of lameness), and is also a useful tool to evaluate up through the fetlock and cannon bone regions. MRI is the gold standard to evaluate bone injuries, soft tissue edema, navicular structures, and certain small ligaments, such as the collateral ligaments of the coffin joint. The superior detail of soft tissues and bone images via MRI provides superior cross-sectional views of the affected anatomy in multiple planes. All MRI studies are reviewed by world-renowned board certified radiologists and are instantly available to veterinarians online.
Nuclear Scintigraphy is commonly referred to as a “bone scan,” and is used to evaluate complex lameness cases. The most common use of a bone scan is in those cases in which nerve blocks fail to localize a lameness, or in which lameness is localized but x-ray findings are negative. Under light sedation, the horse is injected with a rapidly-decaying radioactive isotope that circulates through the body and attaches to areas in which bones are actively remodeling. A scintillation camera and computer then measure the radiation that has temporarily bonded to limbs or vertebrae, discovering what is referred to as “hot spots.” A bone scan can often find a hot spot in its earliest stages, before a lesion has developed sufficiently to appear on x-ray, and when treatment can be most effective.
Regenerative medicine is the treatment of choice for many musculoskeletal problems. B.W. Furlong and Associates is proud to offer Platelet-rich Plasma (PRP) and Stem Cell therapies for both soft tissue and joint problems, either as an immediate treatment for acute injuries, or to maintain performance in chronic cases. Our PRP system is the Magellan Harvest system, which is the same machine used for human care, which guarantees the service is of the highest quality available. The practice also offers IRAP (Interleukin-1 Receptor Antagonist Protein), a unique gene therapy, used in the treatment of joint inflammation, arthritis, and certain soft tissue injuries.
Shock Wave Therapy
Shock wave therapy has proven highly successful in the treatment of soft tissue structures that are close to bone, such as suspensory ligaments, tendons, and feet. It is a commonly used treatment for sport horse injuries because it requires little preparation (light sedation), is non-invasive, and can be used for both chronic and acute injuries. The wave emitted from the device stimulates new cell growth and improves circulation, which promotes healing. The treatment also results in a temporary analgesic effect (pain relief).
B.W. Furlong and Associates has the facilities and highly experienced staff to train stallions to the phantom, regardless of size and temperament. Semen can be collected for fresh –cooled transport or frozen and stored for future use. Stallions may be left at the stallion station while they are trained, evaluated and collected or can be transported to the clinic on a daily basis. The practice also offers access to premiere sporthorse stallions through Shelbourne Farm Stallion Agency.
B.W. Furlong and Associates offers a full range of reproductive services for broodmares throughout the year. During breeding season, our veterinary staff, led by Dr. Gray, use both fresh and frozen semen for artificial insemination and performs broodmare synchronization and preparation for embryo transfer. The practice also provides fetal wellness exams and broodmare-specific vaccinations year-round. Reproductive services can be offered at the farm, in the clinic, or at our satellite breeding facility.
The therapeutic effects of equine acupuncture, which dates back to 2000-3000 BC in China, are produced when specific points, which are areas of increased electrical sensitivity, are stimulated. This results in increased circulation, relief of muscle spasms, improved body defense system, and numerous other benefits. Dr. Wendy Leich is certified in acupuncture therapy and Veterinary Orthopedic Manipulation, a form of animal chiropractic care, and has served as the USET’s 3-Day acupuncturist for the past four Olympic Games. Dr. Jaime Horner & Dr. Misty Gray have also recently become certified as veterinary acupuncturists.
Along with acupuncture, the practice offers chiropractic adjustments as an integrative service. Dr. Gray offers equine chiropractic evaluations and adjustments to enhance the health, comfort, and performance of your equine companion. Chiropractic adjustments are used to restore correct biomechanical motions and nerve functions, and to enhance the body’s innate ability to heal and maintain wellness.
Regenerative Laser Therapy
High Intensity Laser Therapy (HILT) allows for high power outputs of deep penetrating energy to be applied to injuries without the risk of damaging superficial tissues. The wavelength of the laser is optimized for specific tissue types to be treated, while also being minimally absorbed by water, melanin, and hemoglobin. This results in dilation of local vessels and the creation of new vessels, increased availability of growth factors and oxygen, and increases energy production of cells. Furthermore, this laser can recruit and direct certain stem cells to orchestrate healing, which is especially in important in chronic injury and the reduction of scar tissue. The laser can be used in acute injury as well as incorporated in the management of chronic conditions.
Routine dental equilibration, also known as floating, is offered in conjunction with full oral exams to ensure that our patients have full, healthy teeth. These services are offered primarily by Dr. Olson, who uses precise and efficient power dentistry to offer the best services. As issues arise the practice also offers routine dental radiology and advanced procedures.
Gastric ulcers are present in the majority of performance horses, and clinical signs can vary from change in performance, to weight loss, to recurrent colic. The presence of gastric ulcers is confirmed by examining the stomach with gastroscopy, which requires passage of a thin tube down into the horse’s stomach. The video endoscope allows clear visualization of all parts of the stomach, and the images are shown on a large monitor for close examination.
The veterinarians at B.W. Furlong & Associates are dedicated to providing the highest quality of veterinary care for our patients, regardless of the time of day. We now have a full-time vet primarily providing emergency services, allowing us to offer continuity in our emergency care, as well as a quick response time. Colic evaluation, treatment of injuries, and any other emergency service are most commonly provided, and more complicated cases can be admitted into the hospital 24 hours a day.
The laboratory at B.W. Furlong and Associates offers full service in-house laboratory diagnostics. Testing includes complete blood counts, chemistries, blood gases, electrolytes, packed cell volume and total protein. Reference laboratories like Cornell University and Antech Diagnostics are used for more extensive diagnostics. The in-house laboratory also features microscopic diagnostics, available for veterinarians to evaluate cytology on samples like blood, cornea scrapings, uterine biopsies, cultures and semen morphology.
Our practice provides year round ambulatory veterinary support at national and international events, with a focus on the Mid-Atlantic area and Florida. In addition, our services are available by request around the globe to address all your pre-purchase and performance needs.
